I /We understand, acknowledge and agree that, as a result of my/our purchasing and taking up the policy to be issued by insurance company,Thornton Global Wealth Management ("Thornton"),an authorized insurance broker, will be remunerated for its services including renewal, for arranging the said policy. Where the Proposed Policy Owner/Policy Owner is a body corporate, the authorized person who signs on behalf of Proposed Policy Owner/Policy Owner further confirms Thornton that he or she is authorized to do so. My / Our agreement to proceed with this insurance transaction shall constitute my / our consent for Thornton to receive commission and also to collect my/our insurance application and policy related information from insurers.

本人/我們明白、確知及同意就本人/我們購買及接受保險公司簽發的保單，於保單有效期內,包括續保期,本人授權保險經紀,豐盛環球理財("豐盛")收取佣金作為提供保險經紀服務的報酬。假如本保單之準保單持有人/保單持有人為法人團體，代表準保單持有人/保單持有人簽署的授權人員已向豐盛確認他/她已獲該法人團體授權。本人/我們亦同意進行是項保險交易,即構成本人/我們同意豐盛收取佣金，並從保險公司獲取本人/我們投保信息和保單相關資訊。
《Privacy Policy Statement私隱政策聲明》
I/We hereby declare, agree and consent Thornton is authorized to collect, keep and use all of my /
our personal data that have been submitted by me / us to the insurers that introduced by Thornton
for the purposes of processing this application and/or providing subsequent insurance-related
services ( including but not limited to administering the insurance policy and/or processing any
claim and/or data matching) and promoting other products, services or offers to me / us.
本人/我們已授權豐盛向保險公司收集、持有及使用 本人/我們經豐盛介紹的相關產品保險公司所提交的全部個人資料,
以便處理本人/我們申請及/或提供與保險有關的售後服務(包括但不限於保單管理,及/或索償處理及/或資料核對)及推廣其他產品、服務或優惠予 本人/我們之用。
Without prejudice to the aforesaid, my/our personal data may be transferred and disclosed to:
在不減損上述權益的原則下, 本人/我們的個人資料可向下列項目轉移與披露予:
(a) any memeber of the group;
任何豐盛集團成員;
(b) any association or professional bodies in respect of the type of business carried on by Thornton;
任何跟豐盛經營業務種類有關之社團或專業團體;
(c) any intermediary or other service provider rendering services to Thornton in connection with the operation of Thornton's business; operation of IAM's business;
任何向豐盛提供與豐盛業務營運有關的服務中介人或其他服務供應商;
(d) any third party with which I/we have or propose to have dealings; and
任何本人/我們正在或打算進行交易之第三者;及
(e) any government bodies or regulatory authorities.
政府及監管機構。
在不減損上述權益的原則下, 本人/我們的個人資料可向下列項目轉移與披露予:

My / Our agreement to proceed with this insurance transaction shall constitute my / our consent to
the above “Privacy Policy Statement” of Thornton. I/We further understand that the above consent is
necessary for Thornton to process the application
本人/我們同意進行是項保險交易，即構成 本人/我們同意豐盛上述之《私隱政策聲明》。 本人/我們明白必須取得本人/我們以上的同意,才可以處理其保險申請。
